living will, designation of health care surrogate

Pre-need Guardian

Preneed documents are a vital part of the estate planning process.

Designation of health care surrogate (DHCS) is a document that designates one or more people to make health care decision on your behalf. If you are an adult and unmarried, this is a very important document.

Living will, not to be mistaken with a last will and testament, typically informs health care providers that you do not want to be kept alive by use of medical devices when you are in a persistent vegetative state with no chance of recovery.

Durable power of attorney (DPOA) gives someone the authority to act on your behalf for virtually everything. This document can be limited, as to the powers you give someone, and it can continue in existence after you are no longer competent. This document can be very helpful for someone suffering from dementia or Alzheimer’s disease, but it should only be given to someone that is completely trustworthy.
